Brain Health and Cognitive Decline: Expanding the Focus Beyond Glycemic Control in Diabetes Mellitus

Diabetes mellitus (DM) is widely recognized as a major public health problem, with treatment strategies traditionally focused on maintaining glycemic control to prevent vascular complications. In recent years, growing attention has been directed toward the impact of diabetes on brain health. Individuals with diabetes are at a higher risk of experiencing cognitive decline, ranging from mild cognitive impairment to more severe forms of dementia, including Alzheimer’s disease and vascular dementia. Despite this growing body of evidence, cognitive health remains an often-overlooked aspect of diabetes management.

The link between diabetes and cognitive dysfunction is complex and involves multiple biological pathways. Chronic hyperglycemia, insulin resistance, oxidative stress, vascular damage, neuroinflammation, and mitochondrial dysfunction have all been implicated in the development of neuronal injury and impaired cognitive performance. These changes may accelerate brain aging and negatively affect memory, learning, and executive functioning. In addition, lifestyle-related factors such as physical inactivity, poor sleep quality, depression, and socioeconomic challenges can further influence cognitive outcomes in people living with diabetes.

Given these concerns, there is a need to move beyond a glucose-centered approach and adopt a broader perspective that includes the preservation of cognitive health. Early cognitive assessment, effective management of cardiovascular risk factors, promotion of healthy lifestyle behaviors, and attention to mental well-being should be integrated into routine diabetes care. Emerging therapeutic strategies targeting neuroinflammation and impaired insulin signaling may also provide new opportunities to reduce the risk of cognitive decline.

A more comprehensive approach to diabetes management that recognizes the importance of brain health may help improve quality of life, maintain independence, and support healthy aging. Addressing cognitive health alongside metabolic control is essential for reducing the long-term burden of diabetes and its associated complications.
